To the editor:
Just a few short months ago, Hope HealthCare Services announced our "50,000 Wishes for Hope" campaign, a community-wide effort to encourage 10,000 individuals to plan for their future health care needs by completing the advance directives document, "Five Wishes."
I am pleased to share that we have surpassed our goal, with more than 10,000 copies provided free of charge to members of our community.
"Five Wishes" is an easy-to-understand guide that helps each individual to consider and discuss decisions that often need to be made when a serious illness or incident occurs.
The booklet explains possible medical and legal issues in a manner that allows a person to explore personal, spiritual and family preferences, including who should make medical decisions for you, the kind of medical treatment you want, and what you want your loved ones to know.
We thank everyone who supported the "50,000 Wishes for Hope" campaign by requesting a copy, telling others about the booklet or making "Five Wishes" available to employees, colleagues, parishioners and family members.
Hope is pleased to continue making copies available at no charge. Simply contact us at 855-454-3100 or firstname.lastname@example.org.
