Healthy Lee, this month, launched a vital initiative aimed at assessing, understanding, and addressing the health needs of the community and the related social drivers of health (SDOH).

Your involvement will significantly contribute to the success of this effort and Healthy Lee’s collaborative impact to improve the health and well-being of the community.

Healthy Lee is gathering input from the community through this community survey. This quantitative assessment will be supplemental to the current Community Health Needs Assessment underway for 2023. Lee Health asks for community participation and support. They will share the data collected with the East Lee County Chamber of Commerce and the broader community at the end of the survey.
